Melioidosis

Last edited: 4/14/2026

Overview

Melioidosis, caused by the Gram-negative bacterium Burkholderia pseudomallei, presents with diverse clinical manifestations including abscesses, pneumonia, and sepsis, particularly in tropical and subtropical regions 134.

Diagnosis

  • Clinical Presentation: Fever, pneumonia, abscesses, and sepsis 139.
  • Laboratory Tests: Blood cultures are crucial; elevated ferritin levels may indicate severe disease 1.
  • Imaging: Chest X-rays and abdominal imaging to detect abscesses and organ involvement 23.
  • Serology: Useful for exposure assessment but not diagnostic on its own 4.
  • Management

  • First-Line Antibiotics: Combination therapy with ceftazidime or meropenem, often supplemented with doxycycline and trimethoprim-sulfamethoxazole 56.
  • Adjunctive Therapies: Recombinant human activated protein C (rhAPC) may be considered in severe cases with septic shock 5.
  • Supportive Care: Intensive care unit (ICU) support, including mechanical ventilation and management of organ dysfunction 58.
  • Special Populations

  • Pediatrics: High mortality rates observed, especially without identifiable risk factors; hyperferritinemic sepsis-induced multiple-organ dysfunction syndrome is a significant predictor 1.
  • Comorbidities: Diabetes mellitus and alcoholism increase susceptibility and mortality 9.
  • Key Recommendations

  • Early Diagnosis and Aggressive Treatment: Initiate broad-spectrum antibiotics promptly in suspected cases, especially in endemic regions (Evidence: Strong 135).
  • Monitor for Organ Dysfunction: Regularly assess for signs of multiple-organ dysfunction, particularly in pediatric patients and those with hyperferritinemia (Evidence: Moderate 1).
  • Consider Adjunctive Therapies in Severe Cases: Use rhAPC in patients with severe septic shock unresponsive to conventional therapy (Evidence: Weak 5).
  • Supportive ICU Care: Provide intensive supportive care for patients with severe melioidosis, including management of septic shock and respiratory failure (Evidence: Moderate 58).
