Injury of nerve of shoulder region

Last edited: 4/17/2026

Overview

Injury to the nerves of the shoulder region, particularly the axillary nerve, can result from traumatic events or surgical procedures, leading to significant functional impairment including weakness and sensory loss in the deltoid region. 4

Diagnosis

  • Clinical Presentation: Pain, weakness in abduction, and sensory changes over the lateral aspect of the arm. 4
  • Imaging: MRI and MR arthrography can help identify associated structural injuries like labral tears or fractures. 537
  • Electromyography (EMG): Useful for assessing nerve function and confirming axonal damage. 4
  • Physical Examination: Assess deltoid muscle function (e.g., shoulder abduction against resistance) and sensory testing over the lateral arm. 4
  • Management

  • Conservative Management: Rest, physical therapy focusing on shoulder stabilization and gradual strengthening exercises. 4
  • Surgical Intervention: Indicated for severe axonal damage or when conservative measures fail; may include nerve repair or decompression. 4
  • Pain Management: Analgesics (e.g., NSAIDs) for pain control; opioids may be considered for severe pain under close monitoring. 4
  • Rehabilitation: Gradual return to activities under supervision, emphasizing proprioception and motor relearning. 4
  • Special Populations

  • Pediatrics: Avulsion injuries are rare but require careful management to avoid growth disturbances; conservative treatment is often preferred. 33
  • Elderly: Increased risk of comorbidities affecting recovery; tailored rehabilitation programs are crucial. 22
  • Key Recommendations

  • Early MRI Evaluation: To identify associated injuries and guide comprehensive treatment planning. (Evidence: Moderate 537)
  • Physical Therapy Initiation: Early involvement of physical therapy to prevent stiffness and maintain shoulder mobility. (Evidence: Moderate 4)
  • Surgical Consultation for Persistent Symptoms: Consider referral for surgical evaluation if there is no improvement with conservative management after 3-6 months. (Evidence: Expert opinion 4)
  • Avoidance of Improper Vaccine Administration: To prevent iatrogenic injuries like SIRVA, ensuring proper injection technique is crucial. (Evidence: Expert opinion 3439)
