Hydronephrosis

Last edited: 4/13/2026

Overview

Hydronephrosis refers to the dilation of the renal pelvis and calyces due to obstruction of urine flow, often secondary to anatomical abnormalities or calculi. It can be diagnosed antenatally and requires careful management to prevent renal damage 1251425.

Diagnosis

  • Ultrasound: Primary imaging modality for diagnosing hydronephrosis, including antenatal detection 1145560.
  • Grading Systems: Utilization of Society for Fetal Urology (SFU) grading system; improved accuracy through AI and e-learning tools 31323.
  • Postnatal Imaging: Additional imaging like DMSA scans or VCUG for anatomical details 125.
  • Hydronephrosis Index (HI): Quantitative measure for tracking disease progression 28.
  • Management

  • Observation: For mild cases, close monitoring without immediate intervention 1825.
  • Antibiotic Prophylaxis: Variable use in postnatal management; practice patterns vary widely 1527.
  • Surgical Intervention: Indicated for significant obstruction or recurrent issues; includes open surgery and laparoscopy 425.
  • Endourological Procedures: Ureterorenoscopy for calculi, percutaneous nephrostomy for decompression 162248.
  • Medical Expulsive Therapy: Use of alpha-blockers to facilitate stone passage 26.
  • Special Populations

  • Pediatrics: Focus on early detection and conservative management initially; surgical interventions guided by severity 11325.
  • Pregnancy: Conservative management with ureteral stent placement considered for recurrent hydronephrosis 46.
  • Elderly: Considerations for comorbidities and surgical risks; less emphasis on specific interventions noted 20.
  • Key Recommendations

  • Utilize ultrasound for initial diagnosis of hydronephrosis, with SFU grading system for severity assessment (Evidence: Moderate 1313).
  • Implement AI and e-learning tools to enhance accuracy and consistency in grading hydronephrosis (Evidence: Moderate 21323).
  • Tailor management based on severity; consider antibiotic prophylaxis and surgical intervention based on clinical progression (Evidence: Moderate 152725).
  • For pregnant patients with hydronephrosis, prioritize conservative management strategies including ureteral stent placement if necessary (Evidence: Expert opinion 46).
  • Regular monitoring and individualized care plans are crucial in pediatric patients to prevent long-term renal damage (Evidence: Moderate 125).
