Intracranial hypotension

Overview

Diagnosis

  • Clinical Presentation: Postural headaches, neck pain, nausea, vomiting, cranial nerve palsies, and possible radiculopathy.
  • Imaging: MRI or CT myelography to visualize CSF leaks or spinal cord sagging.
  • Lumbar Puncture: CSF opening pressure typically <150 mm H2O.
  • Dynamic Imaging: MRI with Valsalva maneuver to detect CSF volume changes.

  • Management

  • Conservative Treatment: Bed rest, hydration, caffeine, and increased intracranial pressure through NaCl infusions.
  • Epidural Blood Patch: For refractory cases, injecting autologous blood into the epidural space to seal CSF leaks.
  • Surgical Intervention: Rarely needed, involving repair of CSF leaks if conservative measures fail.
