Leiomyosarcoma

Last edited: 4/14/2026

Overview

Leiomyosarcoma is a malignant tumor arising from smooth muscle cells, commonly found in soft tissues but also reported in various locations including skin, gastrointestinal tract, uterus, and rarely in unusual sites such as the pulmonary artery, ligamentum teres of the liver, and saphenous vein. 1231315202636

Diagnosis

  • Clinical Presentation: Varies by location; nonspecific symptoms often include mass formation, pain, and changes in bowel habits. 2293235
  • Imaging: CT, MRI, and ultrasound useful for tumor localization and staging. 22932
  • Biopsy: Essential for definitive diagnosis; fine needle aspiration cytology can be diagnostic in some cases. 51213
  • Histological Features: Presence of spindle cells with cigar-shaped nuclei, eosinophilic cytoplasm, and mitotic activity crucial for grading. 12172735
  • Immunohistochemistry: Useful for confirming smooth muscle origin; reactivity with smooth muscle markers like SMA. 1225
  • Grading: Based on mitotic index and tumor necrosis; low-grade (<10 mitoses/10HPF), intermediate-grade (10-19 mitoses/10HPF), high-grade (≥20 mitoses/10HPF). 1735
  • Special Considerations: Electron microscopy and cytokeratin studies may aid in diagnosis in atypical cases. 1225
  • Management

  • Surgical Excision: Primary treatment; extent depends on tumor location and stage (wide local excision, radical resection). 127142733
  • Mohs Micrographic Surgery: Promising for superficial lesions, offering precise tumor removal. 1
  • Adjuvant Therapy: Chemotherapy (e.g., doxorubicin, ifosfamide) and radiotherapy considered for high-grade tumors or metastatic disease. 182330
  • Targeted Therapy: Limited evidence; emerging role in specific genetic subtypes. 18
  • Follow-Up: Regular imaging and clinical assessment due to risk of recurrence and metastasis. 1830
  • Multidisciplinary Approach: Collaboration with oncologists, surgeons, and radiologists recommended for comprehensive care. 18
  • Special Populations

  • Pregnancy: Limited data; management typically involves close monitoring and individualized surgical intervention post-delivery. 18
  • Pediatrics: Extremely rare; treatment similar to adults but with emphasis on preserving function and minimizing morbidity. 10
  • Elderly: Consider comorbidities and functional status; tailored treatment plans balancing efficacy and tolerability. 18
  • Comorbidities: Presence of other malignancies (e.g., von Recklinghausen's neurofibromatosis) may influence management strategies. 34
  • Key Recommendations

  • Primary Treatment: Wide local excision or radical resection for localized leiomyosarcoma (Evidence: Strong 1214).
  • Adjuvant Therapy for High-Grade Tumors: Consider chemotherapy and radiotherapy for high-grade leiomyosarcomas to improve outcomes (Evidence: Moderate 1830).
  • Regular Follow-Up: Essential for monitoring recurrence and metastasis, especially in high-risk patients (Evidence: Moderate 1830).
