Neutropenic sepsis

Last edited: 4/23/2026

Overview

Neutropenic sepsis is a life-threatening condition characterized by a severe infection in individuals with neutropenia, leading to systemic inflammatory response and potential organ dysfunction 1.

Diagnosis

  • Clinical signs of infection (fever, chills, hypotension) in the context of neutropenia
  • Blood cultures to identify the causative pathogen 5
  • Elevated inflammatory markers (e.g., CRP, procalcitonin) 1070
  • Organ dysfunction assessment using scoring systems like SOFA 11
  • Management

  • Early broad-spectrum antibiotics tailored to local resistance patterns 15
  • Source control (e.g., surgical intervention if applicable) 1
  • Antifungals if fungal infection suspected 5
  • Vasopressors for hypotension (e.g., norepinephrine) 330
  • Fresh frozen plasma (FFP) within 24 hours if coagulopathy present 7
  • Supportive care including fluid resuscitation, inotropic support, and monitoring for organ dysfunction 930
  • Special Populations

  • Pediatrics: Tailored antibiotic therapy and close monitoring of organ function 16
  • Elderly: Increased vigilance for complications like atrial fibrillation and coagulopathy 17
  • Comorbidities: Consider specific management adjustments for coexisting conditions like heart failure 60
  • Key Recommendations

  • Initiate broad-spectrum antibiotics within one hour of recognizing sepsis 1 (Evidence: Strong)
  • Implement early goal-directed therapy including fluid resuscitation and vasopressor support as needed 330 (Evidence: Moderate)
  • Source control measures should be pursued promptly if an infectious focus is identified 1 (Evidence: Strong)
  • Consider fresh frozen plasma transfusion within 24 hours in patients with coagulopathy 7 (Evidence: Moderate)
  • Closely monitor and manage organ dysfunction using scoring systems like SOFA 11 (Evidence: Moderate)
