Adolescent nephronophthisis

Last edited: 4/14/2026

Overview

Nephronophthisis in adolescents is a rare genetic disorder characterized by progressive renal failure due to tubulo-interstitial disease, often presenting with chronic kidney disease and associated complications such as hypertension and anemia. 3640

Diagnosis

  • Clinical presentation includes chronic kidney disease, polyuria, polydipsia, and fatigue.
  • Genetic testing for mutations in genes associated with nephronophthisis (e.g., NPHP1, INVS).
  • Renal ultrasound showing small kidneys with reduced corticomedullary differentiation.
  • Laboratory tests: elevated serum creatinine, decreased glomerular filtration rate (GFR), and electrolyte imbalances. 3640
  • Management

  • Supportive care: Management of hypertension, anemia, and electrolyte imbalances.
  • Dietary modifications: Low-protein diet to reduce uremic symptoms.
  • Growth factors: Erythropoietin for anemia management. 3640
  • Regular monitoring: Frequent follow-ups to assess renal function and adjust treatments as needed.
  • Special Populations

  • Pediatrics: Early diagnosis and multidisciplinary care are crucial due to the chronic nature of the disease and its impact on growth and development. 36
  • Comorbidities: Management of associated conditions like hypertension and anemia requires tailored approaches, often necessitating close collaboration with specialists. 36
  • Key Recommendations

  • Genetic testing for suspected cases to confirm diagnosis and identify specific mutations (Evidence: Moderate 36).
  • Multidisciplinary care involving nephrologists, pediatricians, and geneticists to address complex needs (Evidence: Expert opinion 36).
  • Regular monitoring of renal function and nutritional status to manage complications effectively (Evidence: Moderate 36).
  • Supportive therapies such as erythropoietin for anemia and dietary management to mitigate uremic symptoms (Evidence: Moderate 36).
  • Confidentiality and adolescent-centered care to ensure engagement and adherence to treatment plans (Evidence: Expert opinion 1516).
