Extensor Indicis Tendinitis

Overview

Extensor indicis tendinitis is an overuse injury affecting the extensor indicis proprius (EIP) tendon, which extends the index finger 1. It typically results from repetitive forceful extension or deviation of the index finger, often seen in athletes or individuals with specific occupational demands 1.

Diagnosis

  • Pain and tenderness localized over the distal third of the radius and the dorsal aspect of the metacarpophalangeal (MCP) joint of the index finger 1.
  • Pain exacerbated by resisted extension of the index finger 1.
  • Pain may also occur with passive flexion of the index finger 1.
  • Imaging such as ultrasound or MRI may reveal tendon thickening or inflammation, but diagnosis is primarily clinical 1.
  • Management

  • First-line:
  • * Activity modification to avoid aggravating movements 1. * Rest and ice application 1. * Nonsteroidal anti-inflammatory drugs (NSAIDs) for pain and inflammation 1.
  • Adjunctive:
  • * Stretching and strengthening exercises for the EIP tendon and surrounding muscles 1. * Splinting or bracing to immobilize the index finger 1. * Physical therapy referral for guided rehabilitation 1. * Corticosteroid injections may be considered for persistent symptoms 1.
