Closed extraarticular fracture of distal radius

Last edited: 4/10/2026

Overview

A distal radius fracture is a break in the distal part of the radius bone in the forearm. Closed extraarticular fractures are those that do not involve the joint surface and are treated without surgical intervention.

Diagnosis

  • Radiographs of the wrist, including anteroposterior and lateral views, are the primary diagnostic imaging modality 1.
  • Computed tomography (CT) may be considered for complex fractures or when intra-articular extension is suspected 1.
  • Management

  • Nonoperative management is indicated for stable, closed, extraarticular distal radius fractures 1.
  • Closed reduction and casting are the mainstay of nonoperative treatment 1.
  • Pain management may include oral analgesics 1.
  • Early range of motion exercises are recommended once stability is achieved 1.
  • Key Recommendations

  • For patients with acute distal radius fractures, consider nonoperative management for stable, closed, extraarticular fractures 1. (Evidence: Strong)
  • For patients with acute distal radius fractures, closed reduction and casting are recommended for displaced fractures amenable to acceptable closed reduction 1. (Evidence: Strong)
  • For patients with acute distal radius fractures, early mobilization is recommended to improve functional outcomes 1. (Evidence: Moderate)
