Overview

Dementia is a syndrome characterized by a decline in cognitive function severe enough to impair daily life and independent living. It encompasses various neurodegenerative diseases, with Alzheimer's disease being the most common.

Diagnosis

  • Cognitive assessments (e.g., MMSE, MoCA) 67
  • Neuropsychological testing to identify specific cognitive deficits 6
  • Brain imaging (MRI, CT) to rule out other causes and identify structural changes 6
  • Laboratory tests to exclude reversible causes (e.g., vitamin deficiencies, infections) 6
  • Biomarker analysis (CSF, blood) for specific types like Alzheimer's disease 15
  • Management

  • Non-pharmacological Interventions: Cognitive stimulation, occupational therapy, and social engagement 2622
  • Pharmacological Treatments:
  • - Cholinesterase inhibitors (e.g., donepezil, rivastigmine, galantamine) for mild to moderate Alzheimer's disease 6 - Memantine for moderate to severe Alzheimer's disease 6
  • Behavioral Management: Non-pharmacological approaches for behavioral symptoms 26
  • Caregiver Support: Interventions to reduce psychological distress 3
  • Special Populations

  • Elderly: Increased risk of polypharmacy and drug interactions; careful medication review recommended 81173
  • Comorbidities: Management of cardiovascular risk factors (e.g., blood pressure control) to potentially mitigate dementia progression 143336
  • Polypharmacy: High prevalence; deprescribing initiatives to reduce medication burden 975
  • Key Recommendations

  • Implement cognitive assessments and neuropsychological testing for accurate diagnosis 67 (Evidence: Strong)
  • Utilize non-pharmacological interventions such as cognitive stimulation and occupational therapy to improve quality of life 26 (Evidence: Strong)
  • Consider pharmacological treatments like cholinesterase inhibitors and memantine based on disease severity 6 (Evidence: Strong)
  • Provide caregiver support interventions to alleviate psychological distress 3 (Evidence: Strong)
  • Regularly review and manage polypharmacy in elderly patients with dementia to minimize adverse effects 873 (Evidence: Moderate)
  • Control cardiovascular risk factors, including blood pressure management, to potentially slow cognitive decline 1433 (Evidence: Moderate)
