Primary idiopathic dilated cardiomyopathy

Overview

Idiopathic dilated cardiomyopathy (IDC) is a primary myocardial disorder characterized by left ventricular dilatation and impaired systolic function, often leading to heart failure symptoms 12.

Diagnosis

  • Clinical Presentation: Symptoms include dyspnea, fatigue, and exercise intolerance 12.
  • Electrocardiography (ECG): May show nonspecific changes like left ventricular hypertrophy or conduction abnormalities 68.
  • Echocardiography: Essential for assessing left ventricular ejection fraction (LVEF), end-diastolic volume (LVEDV), and presence of mitral regurgitation 210.
  • Cardiac MRI: Provides detailed assessment of myocardial structure, fibrosis, and right ventricular function 24.
  • Prognostic Indicators: Right ventricular dysfunction, elevated serum uric acid levels, and QRS prolongation are associated with worse outcomes 1223.
