Megaloblastic anemia

Overview

Megaloblastic anemia is characterized by ineffective hematopoiesis resulting in macrocytic anemia, often due to deficiencies in vitamin B12 or folate. It presents with pancytopenia and distinctive nuclear and cellular changes in blood and bone marrow. 624

Diagnosis

  • Clinical Presentation: Pancytopenia, macrocytic anemia, glossitis, and neurological symptoms. 6
  • Laboratory Findings: Elevated mean corpuscular volume (MCV), hypersegmented neutrophils, and low serum vitamin B12 or folate levels. 624
  • Bone Marrow Examination: Presence of megaloblasts and hypersegmented nuclei in myeloid precursors. 6
  • Serum Biomarkers: Elevated methylmalonic acid and homocysteine levels can support the diagnosis, especially when B12 levels are borderline. 24
  • Management

  • Vitamin Supplementation:
  • - Vitamin B12: Intramuscular injections of hydroxocobalamin or cyanocobalamin, typically 1 mg weekly for several weeks, then monthly. 620 - Folate: Oral or parenteral supplementation depending on deficiency severity; doses vary but often start at 1 mg daily. 620
  • Dietary Modifications: Encourage intake of vitamin B12-rich foods (meat, fish, dairy) and folate-rich foods (leafy greens, legumes). 20
  • Monitoring: Regular blood counts and vitamin levels to assess response and adjust treatment. 6
  • Special Populations

  • Pregnancy: Routine folate supplementation is recommended throughout pregnancy, not just preconception and first trimester, to prevent severe folate deficiency leading to megaloblastic anemia. 20
  • Pediatrics: Megaloblastic anemia due to vitamin B12 or folate deficiency is rare in infants but can occur; early diagnosis and supplementation are crucial. 9
  • Elderly: Increased vigilance for vitamin deficiencies due to malabsorption syndromes or inadequate dietary intake; regular screening may be necessary. 6
  • Comorbidities: Patients on triamterene may require monitoring for folate deficiency, as it can induce megaloblastic anemia. 22

  • Initiate Vitamin B12 Supplementation for confirmed deficiency with intramuscular injections of 1 mg weekly initially, then monthly, to treat megaloblastic anemia. (Evidence: Strong 6)
