Coronary arteriosclerosis

Last edited: 4/23/2026

Overview

Coronary arteriosclerosis involves the hardening and narrowing of coronary arteries due to plaque buildup, increasing the risk of myocardial infarction and ischemic heart disease. It is a significant contributor to cardiovascular morbidity and mortality.

Diagnosis

  • Key Diagnostic Criteria: Elevated pulse wave velocity (PWV), increased arterial stiffness, and presence of atherosclerotic plaques on imaging.
  • Recommended Tests:
  • - Pulse wave velocity measurement (e.g., brachial-ankle PWV) 12416 - Carotid arterial strain values 17 - Imaging techniques (e.g., MRI, CT angiography) to assess plaque burden and arterial structure 33
  • Grading: Use PWV thresholds (e.g., baPWV > 14 m/s indicative of increased stiffness) 124
  • Management

  • First-Line Treatments:
  • - Lifestyle modifications (increased physical activity, dietary changes) 12932 - Blood pressure control with ACE inhibitors or ARBs (e.g., valsartan) 3839
  • Adjunctive Treatments:
  • - Statins for lipid management 332 - Antiplatelet therapy (e.g., aspirin) 32 - Management of comorbidities (e.g., diabetes, hypertension) 314

    Special Populations

  • Elderly: Increased arterial stiffness and higher risk of complications; focus on comprehensive risk factor management 130
  • Comorbidities: Hypertension and obesity synergistically exacerbate arteriosclerosis 2644
  • Pregnancy: Limited specific data; manage hypertension and other risk factors cautiously 59
  • Key Recommendations

  • Implement regular physical activity to reduce arterial stiffness 129 (Evidence: Moderate)
  • Control blood pressure using ACE inhibitors or ARBs to mitigate progression of arteriosclerosis 3839 (Evidence: Moderate)
  • Use statins for lipid management to decrease atherosclerotic burden 332 (Evidence: Strong)
  • Monitor and manage inter-arm blood pressure differences as a marker for cardiovascular risk 7 (Evidence: Moderate)
  • Consider dietary interventions targeting arterial stiffness, such as reducing oxidative stress 2243 (Evidence: Moderate)
