Felty's syndrome

Overview

Felty's syndrome is a complication of long-standing rheumatoid arthritis characterized by the triad of rheumatoid arthritis, splenomegaly, and recurrent infections, often accompanied by cytopenias. 1

Diagnosis

  • Clinical Criteria: Presence of rheumatoid arthritis, splenomegaly, and at least one of the following: neutropenia, recurrent infections, or lymphadenopathy 1
  • Laboratory Tests: Positive rheumatoid factor, complete blood count (CBC) showing cytopenias (e.g., neutropenia, leukopenia) 1
  • Imaging: Ultrasound or CT scan to confirm splenomegaly 1
  • Management

  • First-Line Treatments:
  • - Splenectomy: Consider for severe neutropenia unresponsive to medical management 1
  • Adjunctive Treatments:
  • - Immunosuppressive Therapy: Use of disease-modifying antirheumatic drugs (DMARDs) such as methotrexate to control rheumatoid arthritis 1 - Antibiotics: Prophylactic or therapeutic use to manage and prevent infections 1

    Special Populations

  • Late Manifestations: Arthritis may develop later in the course of the disease, even without initial clinical signs 1
