Closed fracture of rib

Last edited: 4/10/2026

Overview

Closed fractures of the rib are bone breaks in the rib cage. Management strategies range from non-operative approaches to surgical fixation, particularly for specific patient populations and fracture patterns.

Diagnosis

Management

  • Surgical fixation of rib fractures has demonstrated significant reductions in mechanical ventilation duration, intensive care unit (ICU) and hospital lengths of stay, and rates of pneumonia and tracheostomy compared to non-operative management 1.
  • Non-operative management is the standard for most rib fractures.
  • Special Populations

    Key Recommendations

  • Surgical intervention for rib fractures leads to significantly lower rates of pneumonia, shorter ICU stays, and reduced time on mechanical ventilation 1. (Evidence: Strong)
  • Rib fixation for ventilator-dependent flail chest patients is a mainstay of management in major trauma centers 1. (Evidence: Strong)
  • Benefits of rib fixation in non-ventilator-dependent groups remain largely unproven, with no significant benefit in quality of life at six months identified in a meta-analysis of randomized controlled trials 1. (Evidence: Strong)
