Gestational trophoblastic neoplasia

Last edited: 4/14/2026

Overview

Gestational trophoblastic neoplasia (GTN) encompasses a spectrum of malignant conditions arising from trophoblastic cells, including choriocarcinoma, placental site trophoblastic tumor (PSTT), and epithelioid trophoblastic tumor. These conditions often originate from molar pregnancies and can present with local invasion or distant metastasis. 157

Diagnosis

  • Incidental Diagnosis: Often diagnosed incidentally due to silent presentation.
  • Fetomaternal Hemorrhage (FMH): Linked to intraplacental choriocarcinoma and associated complications like stillbirth and intrauterine growth restriction. 1
  • Histological Confirmation: Post-delivery diagnosis common in intraplacental cases.
  • Serum hCG Levels: Monitoring serum human chorionic gonadotropin (hCG) levels crucial for diagnosis and treatment monitoring. 6
  • FIGO Scoring System: Utilizes risk factors and staging to categorize patients into low-risk or high-risk groups. 78
  • Management

  • First-Line Chemotherapy:
  • - High-Risk GTN (FIGO Score ≥5): Floxuridine, Actinomycin D, Etoposide, and Vincristine (FAEV) regimen shows efficacy with 85.3% complete remission rate. 3 - Standard Regimens: 5-fluorouracil-based multiagent chemotherapy remains common in many settings.
  • Adjunctive Treatments:
  • - Metastatic Disease: Multidisciplinary approach including surgery, radiation, and further chemotherapy as needed. - Toxicity Management: Close monitoring for neutropenia, thrombocytopenia, and anemia, especially with FAEV regimen. 3

    Special Populations

  • Pregnancy: Twin pregnancies with a normal fetus and coexisting GTN require careful management balancing maternal and fetal health. 9
  • Comorbidities: High-risk features like PSTT histology, high FIGO scores, and chemotherapy resistance correlate with poorer outcomes. 5
  • Key Recommendations

  • Utilize FIGO risk scoring system for accurate staging and guiding treatment intensity (Evidence: Strong 78).
  • Employ FAEV regimen as a first-line treatment for high-risk GTN (FIGO Score ≥5) with close monitoring for toxicity (Evidence: Moderate 3).
  • Regularly monitor serum hCG levels for diagnosis and treatment response in GTN management (Evidence: Moderate 6).
  • Consider multidisciplinary approaches for managing metastatic GTN, incorporating surgery and radiation when necessary (Evidence: Expert opinion 5).
  • Address ethical concerns related to equitable access to advanced diagnostic tools like NIPT, especially in resource-limited settings (Evidence: Weak 4).
