Obstruction of larynx

Last edited: 4/14/2026

Overview

Obstruction of the larynx can result from various etiologies including structural abnormalities, inflammatory conditions, tumors, and foreign bodies, leading to significant airway compromise and voice disturbances 22.

Diagnosis

  • Clinical Presentation: Stridor, dysphonia, dyspnea, and difficulty breathing 22.
  • Physical Examination: Direct or indirect laryngoscopy to visualize the larynx and identify obstructing lesions 22.
  • Imaging: CT or MRI of the neck to assess structural abnormalities and tumor extent 22.
  • Flexible Laryngoscopy: Useful in pediatric patients for assessing subglottic structures 7.
  • Management

  • Airway Management: Immediate intervention for acute airway obstruction, potentially including intubation or tracheostomy 22.
  • Surgical Intervention: Removal of foreign bodies, excision of tumors, or correction of structural abnormalities 22.
  • Medical Management: Corticosteroids for inflammatory conditions like laryngitis 22.
  • Voice Therapy: For functional voice disorders and post-surgical rehabilitation 22.
  • Special Populations

  • Pediatrics: Use of appropriately sized cuffed or uncuffed endotracheal tubes based on subglottic diameter assessment 7.
  • Elderly: Focus on managing presbylaryngis with voice therapy and supportive care 22.
  • Comorbidities: Tailor management considering coexisting conditions affecting airway patency 22.
  • Key Recommendations

  • Immediate Airway Assessment and Intervention for acute laryngotracheal obstruction to prevent respiratory failure (Evidence: Strong 22).
  • Utilize Imaging Studies (CT/MRI) to evaluate structural causes of laryngeal obstruction for definitive management planning (Evidence: Moderate 22).
  • Consider Voice Therapy as part of the management plan for chronic dysphonia related to laryngeal obstruction (Evidence: Moderate 22).
  • Select Appropriate Endotracheal Tube Size in pediatric patients based on subglottic airway measurements to avoid obstruction (Evidence: Moderate 7).
  • Incorporate Diagnostic Laryngoscopy early in the evaluation of suspected laryngeal obstructions to guide treatment (Evidence: Moderate 22).
