Supraventricular tachycardia

Last edited: 4/23/2026

Overview

Supraventricular tachycardia (SVT) encompasses a group of rapid heart rhythms originating above the ventricles, often presenting as paroxysmal episodes characterized by abrupt onset and termination. 123411243139

Diagnosis

  • Clinical Presentation: Narrow QRS complex on ECG, often with regular rhythm and heart rate >150 bpm. 142431
  • Electrocardiogram (ECG): Key for identifying narrow complex tachycardia; consider specific patterns like AVNRT or orthodromic reciprocating tachycardia. 2283350
  • Electrophysiological Study: Indicated for refractory cases or to guide ablation; useful for differentiating SVT mechanisms. 9283140
  • Holter Monitoring: Useful for detecting intermittent episodes in asymptomatic patients. 160
  • Management

  • First-Line Treatments:
  • - Valsalva Maneuver: Recommended for acute termination. 64664 - Adenosine: Effective for AVNRT and orthodromic reciprocating tachycardia; caution in patients with asthma or lung disease. 51555
  • Pharmacologic Therapy:
  • - Beta-Blockers: For rate control in stable patients. 11 - Calcium Channel Blockers: Alternative for rate control, particularly verapamil. 11 - Antiarrhythmics: Flecainide or propafenone for maintenance therapy in refractory cases. 1161
  • Catheter Ablation:
  • - Primary Treatment: Recommended for recurrent SVT unresponsive to medication. 9123347 - Techniques: High-density mapping, zero-fluoroscopy approaches, and cryoablation are advancing efficacy and safety. 7263638

    Special Populations

  • Pediatrics:
  • - Insurance Disparities: Admission rates vary by primary insurance payor. 1 - Psychological Impact: Higher risk of developing psychological difficulties compared to healthy peers. 39 - Management: Focus on catheter ablation with advancements in minimizing radiation exposure. 72147
  • Pregnancy:
  • - Gestational SVT: Management includes careful consideration of fetal safety; modified Valsalva maneuver and catheter ablation are options. 2746
  • Elderly:
  • - Increased Comorbidities: Higher incidence of comorbidities like atrial flutter post-ablation; tailored pharmacologic and procedural approaches are essential. 12

    Key Recommendations

  • Catheter Ablation as Primary Therapy: Recommended for recurrent SVT unresponsive to medication (Evidence: Strong 9123347).
  • Use of Valsalva Maneuver for Acute Termination: Effective and safe for terminating SVT episodes (Evidence: Moderate 64664).
  • Minimizing Radiation Exposure in Pediatric Ablation: Employ fluoroscopy-minimizing strategies and zero-fluoroscopy techniques (Evidence: Moderate 72636).
  • Adenosine for AVNRT: First-line pharmacological agent for acute termination, with caution in respiratory conditions (Evidence: Moderate 51555).
  • Monitoring and Psychological Support in Pediatric Patients: Routine monitoring and psychological assessment due to potential long-term effects (Evidence: Expert opinion 39).
