Clear cell acanthoma

Last edited: 4/14/2026

Overview

Clear cell acanthoma (CCA) is a benign epidermal tumor characterized by acanthotic epidermis with clear cells and a distinctive collarette scale 1. It can present in various atypical forms, including cystic, polypous structures, and multiple lesions 25.

Diagnosis

  • Dermoscopic Features: Squamous surface with a translucent collarette, dilated capillary loops oriented perpendicularly to the skin surface, resembling psoriasis-like patterns 1.
  • Histological Criteria: Acanthotic epidermis with clear cells, often showing a collarette of parakeratotic scale 24.
  • Melanocyte Presence: Melanocytes are consistently present but with variable density 4.
  • Management

  • Surgical Excision: Primary treatment for definitive diagnosis and management of solitary lesions 3.
  • Cryotherapy: Effective for multiple or persistent lesions, particularly useful in cases with associated conditions like dry skin and varicose veins 3.
  • Monitoring: For asymptomatic cases, regular monitoring may be sufficient if lesions are solitary and stable 3.
  • Special Populations

  • Comorbidities: Presence of conditions like dry skin and varicose veins may correlate with multiple CCA lesions 3.
  • Treatment Considerations: Cryotherapy may be particularly beneficial in elderly patients with multiple lesions 3.
  • Key Recommendations

  • Utilize dermoscopic findings, particularly psoriasis-like vascular patterns, as supportive evidence for diagnosing CCA (Evidence: Moderate 1).
  • Consider cryotherapy as a viable treatment option for multiple or persistent CCA lesions, especially in patients with associated comorbidities (Evidence: Weak 3).
  • Exclude surgical excision for definitive diagnosis and management of solitary CCA lesions (Evidence: Expert opinion 3).
