Hemangioendothelioma

Last edited: 4/14/2026

Overview

Infantile hemangioendothelioma (IHE) is a rare benign vascular tumor that can present with life-threatening complications, particularly Kasabach-Merritt syndrome characterized by thrombocytopenia and consumptive coagulopathy, especially in kaposiform variants 125.

Diagnosis

  • Clinical Presentation: Large vascular mass, often retroperitoneal or mediastinal, with signs of coagulopathy (thrombocytopenia) 125.
  • Imaging: CT, MRI, and ultrasound are crucial for tumor localization and extent 16.
  • Biopsy: Essential for definitive diagnosis, distinguishing from hemangioma 48.
  • Laboratory Tests: Complete blood count (CBC) to assess thrombocytopenia and coagulation profile 125.
  • Management

  • First-Line Treatment:
  • - Corticosteroids: Often initial therapy for managing Kasabach-Merritt syndrome 35. - Vincristine: Increasingly used in combination with corticosteroids for refractory cases 35.
  • Adjunctive Treatments:
  • - Alpha-Interferon: Used in combination with other agents 1. - Chemotherapy: Other chemotherapeutic agents like vincristine have shown efficacy 13. - Radiation Therapy: Reported in hepatic cases with tumor regression 9.

    Special Populations

  • Pediatrics: Most common in infants, with high morbidity and mortality rates, especially with Kasabach-Merritt syndrome 12456.
  • Comorbidities: Presence of consumptive coagulopathy significantly impacts treatment response and outcomes 25.
  • Key Recommendations

  • Combination Therapy: Use corticosteroids and vincristine as first-line treatment for Kasabach-Merritt syndrome associated with kaposiform hemangioendothelioma (Evidence: Moderate) 35.
  • Aggressive Monitoring: Regular CBC and coagulation profile monitoring essential in managing thrombocytopenia and coagulopathy (Evidence: Expert opinion) 125.
  • Early Intervention: Prompt initiation of therapy is critical due to unpredictable disease progression and potential for severe complications (Evidence: Moderate) 125.
  • Consider Interferon: Addition of alpha-interferon to conventional therapy may enhance efficacy in refractory cases (Evidence: Weak) 1.
  • Multidisciplinary Approach: Involvement of pediatric hematology, oncology, and radiology is recommended for comprehensive management (Evidence: Expert opinion) 1245.
