Ventricular tachyarrhythmia

Last edited: 4/22/2026

Overview

Ventricular tachyarrhythmias (VT) are rapid heart rhythms originating in the ventricles, significantly contributing to sudden cardiac death 1. These arrhythmias can be induced by various factors including myocardial ion channel diseases and psychological stress 119.

Diagnosis

  • Electrocardiogram (ECG): Essential for identifying VT patterns 412.
  • Holter Monitoring: Useful for detecting intermittent VT episodes 22.
  • Electrophysiology Study: Evaluates arrhythmia mechanisms and inducibility 1724.
  • QT Dispersion and Signal-Averaged ECG: Predict susceptibility to VT 13.
  • T Wave Alternans (TWA): Identifies arrhythmic vulnerability 4.
  • Management

  • Implantable Cardioverter-Defibrillator (ICD): Recommended for high-risk patients 818.
  • Antiarrhythmic Drugs:
  • - Beta-Blockers: Reduce VT recurrence 152. - Amiodarone: Effective in treating refractory VT 712. - Sotalol: Comparable efficacy to amiodarone in some studies 1011.
  • Radiofrequency Catheter Ablation: Suitable for specific VT subtypes 8.
  • Magnesium Therapy: Beneficial in normomagnesemic patients with refractory VT 25.
  • Special Populations

  • Elderly: ICD efficacy and safety profiles need careful consideration 18.
  • Comorbidities: Preoperative evaluation crucial for patients with underlying heart disease 23.
  • Key Recommendations

  • ICD Implantation for High-Risk Patients: Recommended for those with sustained VT or aborted cardiac arrest 8 (Evidence: Strong).
  • Beta-Blockers as First-Line Therapy: Effective in reducing VT recurrence in ICD recipients 152 (Evidence: Moderate).
  • Consider Amiodarone for Refractory VT: Particularly useful in patients unresponsive to other antiarrhythmic drugs 712 (Evidence: Moderate).
  • Serial Electrophysiology Studies: Can predict efficacy of serial drug testing in VT management 17 (Evidence: Moderate).
  • Holter Monitoring for VT Surveillance: Useful for detecting asymptomatic VT episodes 22 (Evidence: Moderate).
