Hypertrophy of tonsils AND adenoids

Last edited: 4/14/2026

Overview

Hypertrophy of tonsils and adenoids refers to the enlargement of these lymphoid tissues, often leading to obstructive symptoms such as nasal obstruction, snoring, and sleep-disordered breathing. This condition is commonly evaluated and managed in otolaryngology practice 1212.

Diagnosis

  • Clinical Evaluation: History of obstructive symptoms, snoring, and sleep disturbances 12.
  • Physical Examination: Direct visualization of enlarged tonsils and adenoids 12.
  • Imaging: Flexible nasendoscopy or lateral neck X-rays (for adenoids) to assess size and degree of obstruction 12.
  • Sleep Studies: Polysomnography to evaluate for sleep apnea if obstructive symptoms are severe 12.
  • Management

  • Conservative Management: Watchful waiting for mild cases without significant symptoms 12.
  • Medical Therapy: No specific pharmacological treatments; symptomatic management (e.g., decongestants for nasal obstruction) 12.
  • Surgical Intervention: Tonsillectomy and/or adenoidectomy for persistent symptoms or significant obstruction 1216.
  • Follow-Up: Regular monitoring for symptom progression or resolution 12.
  • Special Populations

  • Pediatrics: Commonly affected; surgical intervention often recommended for persistent symptoms 12.
  • Elderly: Less commonly discussed; management focuses on symptom relief and quality of life 12.
  • Comorbidities: Presence of obstructive sleep apnea may necessitate more aggressive intervention 1212.
  • Key Recommendations

  • Evaluate Symptoms and Physical Findings to determine the need for intervention (Evidence: Moderate 12).
  • Consider Polysomnography for patients with severe obstructive symptoms to assess for sleep apnea (Evidence: Moderate 12).
  • Perform Tonsillectomy and/or Adenoidectomy for persistent symptoms or significant airway obstruction (Evidence: Expert opinion 16).
  • Regular Follow-Up is essential to monitor symptom changes and treatment outcomes (Evidence: Expert opinion 1).
  • Tailor Management Based on Age and Comorbidities, prioritizing quality of life and symptom relief (Evidence: Expert opinion 12).
