Chronic osteomyelitis of multiple sites

Last edited: 4/14/2026

Overview

Chronic osteomyelitis affecting multiple sites is a persistent infection of bone lasting more than six weeks, often complicated by recurrent infections and significant morbidity due to polypharmacy and multiple comorbidities in elderly patients. 1231120

Diagnosis

  • Clinical signs include persistent pain, swelling, and local tenderness.
  • Radiographic imaging (X-rays, MRI) essential for bone involvement and extent.
  • Bone scans or nuclear medicine studies can confirm osteomyelitis activity.
  • Cultures from affected sites (e.g., bone biopsy) crucial for microbiological diagnosis.
  • Laboratory tests (CRP, ESR) to assess inflammation levels. 21120
  • Management

  • Antibiotics: Initial broad-spectrum coverage followed by targeted therapy based on culture results (e.g., vancomycin, rifampin, flucloxacillin). 21120
  • Surgical intervention: Debridement and stabilization for localized disease or refractory cases.
  • Pain management: Analgesics (opioids if necessary) to control symptoms.
  • Addressing polypharmacy: Regular medication reviews to minimize adverse drug interactions and optimize therapy. 31427
  • Wound care: Proper dressing changes and infection control measures.
  • Rehabilitation: Physical therapy to maintain mobility and function. 21120
  • Special Populations

  • Elderly: Increased risk of polypharmacy and adverse drug interactions; tailored medication management crucial. 131427
  • Comorbidities: Patients with multiple chronic conditions require integrated care plans to manage complexity effectively. 121120
  • Key Recommendations

  • Conduct thorough medication reviews to minimize polypharmacy and prevent adverse drug interactions in patients with chronic osteomyelitis across multiple sites. (Evidence: Moderate) 31427
  • Implement targeted antibiotic therapy based on culture and sensitivity results to optimize treatment efficacy and reduce resistance. (Evidence: Strong) 21120
  • Integrate surgical interventions judiciously for cases refractory to medical management to ensure effective local control of infection. (Evidence: Moderate) 21120
  • Prioritize patient-centered care approaches to align treatment with individual patient priorities and reduce treatment burden. (Evidence: Expert opinion) 715
