Overview

Moyamoya disease is a chronic, progressive cerebrovascular disorder characterized by stenosis or occlusion of the internal carotid arteries, leading to the development of compensatory collateral vessels resembling a "puff of smoke" on angiography 144.

Diagnosis

  • Key Diagnostic Criteria: Narrowing of the internal carotid arteries, characteristic "puff of smoke" collateral vessels on cerebral angiography 144.
  • Recommended Tests:
  • - Cerebral angiography (gold standard) 1. - Magnetic resonance vessel wall imaging (MR VWI) for detailed vessel wall assessment 31138. - Contrast-enhanced MR vessel wall imaging to evaluate enhanced sulci 3. - Indocyanine green videoangiography for intraoperative hemodynamic assessment 212.
  • Grading: Modified Suzuki classification for angiographic severity 1.
  • Management

  • First-Line Treatments:
  • - Revascularization surgery (e.g., STA-MCA bypass) 72140. - Direct vs. indirect bypass with considerations for hemodynamic outcomes 5.
  • Adjunctive Treatments:
  • - Antiplatelet therapy (e.g., aspirin) post-bypass to prevent thrombosis 52. - Strict blood pressure control, especially in pediatric patients to reduce stroke risk 8. - Management of cerebral hyperperfusion syndrome with careful intraoperative monitoring and postoperative BP control 218.

    Special Populations

  • Pediatrics:
  • - Close monitoring for hemorrhagic events and strict BP control postoperatively 817.
  • Pregnancy:
  • - Limited evidence; mode of birth should be individualized based on maternal and fetal risks 20.
  • Comorbidities:
  • - Consideration of hereditary spherocytosis association and tailored management 14.

    Key Recommendations

  • Perform cerebral angiography for definitive diagnosis of Moyamoya disease (Evidence: Strong 144).
  • Utilize revascularization surgery (e.g., STA-MCA bypass) as the primary treatment approach (Evidence: Strong 740).
  • Maintain strict blood pressure control postoperatively, particularly in pediatric patients, to mitigate stroke risk (Evidence: Moderate 8).
  • Implement antiplatelet therapy post-bypass surgery to reduce the risk of thrombosis (Evidence: Moderate 52).
  • Intraoperative and postoperative monitoring for cerebral hyperperfusion syndrome is crucial to prevent complications (Evidence: Moderate 218).
