Charcot arthropathy of hindfoot

Last edited: 4/16/2026

Overview

Charcot arthropathy of the hindfoot, also known as neuropathic arthropathy, is a progressive, destructive joint disorder typically associated with peripheral neuropathy, most commonly seen in patients with diabetes mellitus. It leads to significant deformity and functional impairment if not promptly managed 1.

Diagnosis

  • Clinical Presentation: Presence of neuropathy with insidious onset of joint pain, swelling, and deformity 1.
  • Imaging: Radiographic findings include joint space widening, fragmentation of bone, and soft tissue swelling 1.
  • MRI: Useful for assessing early changes and soft tissue involvement 1.
  • Grading Systems: Eichenholtz classification system for severity grading (I-IV) 1.
  • Management

  • Immobilization: Initial management with total contact casts or bracing to stabilize the foot and prevent further deformity 1.
  • Orthotic Devices: Custom orthotics to support the foot and maintain alignment 1.
  • Surgical Intervention: Indicated for severe deformities, joint fusion, or osteotomy to correct alignment 1.
  • Pain Management: Nonsteroidal anti-inflammatory drugs (NSAIDs) for symptomatic relief 1.
  • Infection Prevention: Close monitoring and prophylactic antibiotics if surgical intervention is planned 1.
  • Special Populations

  • Diabetes Mellitus: Primary condition predisposing to Charcot arthropathy; strict glycemic control is crucial 1.
  • Elderly: Higher risk of complications; careful monitoring and tailored rehabilitation are essential 1.
  • Key Recommendations

  • Initiate immobilization with total contact casts early in the course of Charcot arthropathy to prevent deformity progression (Evidence: Strong 1).
  • Regular radiographic monitoring is essential for assessing disease progression and guiding treatment adjustments (Evidence: Moderate 1).
  • Consider surgical intervention for patients with severe deformities that compromise function or lead to ulceration (Evidence: Expert opinion 1).
