Endocarditis of prosthetic tricuspid valve

Last edited: 4/22/2026

Overview

Tricuspid valve endocarditis (TVIE) is an infection involving the tricuspid valve, commonly seen in intravenous drug users, often complicated by high rates of reinfection and associated comorbidities like methicillin-resistant Staphylococcus aureus (MRSA) infections 124.

Diagnosis

  • Clinical triad: Intravenous drug use, Staphylococcus aureus septicemia, and septic pulmonary embolism strongly suggest TVIE 4.
  • Echocardiography: Essential for visualizing vegetations; M-mode and 2-D echocardiography are useful techniques 4.
  • Absence of murmurs: Does not rule out TVIE; vegetations can be present without audible murmurs 4.
  • Blood cultures: Crucial for identifying causative organisms 12.
  • Management

  • First-line treatment: Intravenous antibiotics tailored to the identified organism, often including agents effective against MRSA 12.
  • Surgical intervention: Standard for severe cases, including valve replacement or repair without annuloplasty rings to minimize foreign material 3.
  • Percutaneous Mechanical Aspiration (PMA): Less invasive alternative for high-risk patients; efficacy similar to surgery in terms of in-hospital mortality but higher risk for 30-day mortality 1.
  • Consideration for PWID: PMA may be preferred over valve surgery due to lower risk of reinfection 2.
  • Special Populations

  • Intravenous Drug Users (PWID): Higher risk of reinfection; PMA may be considered over surgical valve replacement 2.
  • Comorbidities: Specific management strategies like tricuspid valve repair without annuloplasty rings to reduce reinfection risk 3.
  • Key Recommendations

  • Use echocardiography for diagnosis of TVIE, even in the absence of characteristic murmurs 4 (Evidence: Moderate).
  • Consider percutaneous mechanical aspiration (PMA) as a less invasive alternative to surgery in high-risk patients, particularly PWID, despite higher 30-day mortality risk 12 (Evidence: Moderate).
  • Prefer tricuspid valve repair over replacement in intravenous drug abusers to minimize reinfection risk 3 (Evidence: Moderate).
  • Tailor antibiotic therapy based on blood culture results, emphasizing coverage for MRSA 12 (Evidence: Moderate).
