Laryngotracheal hemangioma

Last edited: 4/23/2026

Overview

Laryngotracheal hemangiomas are benign vascular tumors that can occur in the larynx and trachea, often presenting with airway obstruction or respiratory symptoms 267.

Diagnosis

  • Clinical Presentation: Obstructive symptoms, stridor, and respiratory distress 2.
  • Imaging: MRI and CT scans are crucial for diagnosis, showing characteristic vascular lesions 265.
  • Endoscopy: Direct visualization can confirm the presence and extent of the lesion 67.
  • Laboratory Tests: Typically not diagnostic but may show signs of complications like anemia or thrombocytopenia in severe cases 45.
  • Management

  • Conservative Management: Monitoring and supportive care for asymptomatic lesions 67.
  • Medications:
  • - Propranolol: First-line treatment for symptomatic lesions, typically starting at 2-3 mg/kg/day, titrated up to 6 mg/kg/day 41444. - Corticosteroids: Used adjunctively, especially in cases with significant inflammation or rapid growth 1444.
  • Interventional Procedures:
  • - Embolization: For severe cases with high-output cardiac failure or life-threatening bleeding 1524. - Laser Therapy: For localized lesions causing significant obstruction 67.

    Special Populations

  • Pediatrics: Propranolol is well-tolerated and effective in infants 41444.
  • Comorbidities: Close monitoring required in cases with associated Kasabach-Merritt syndrome or high-output cardiac failure 4578.
  • Key Recommendations

  • Initiate propranolol for symptomatic laryngotracheal hemangiomas at a dose of 2-3 mg/kg/day, titrating up to 6 mg/kg/day for optimal response (Evidence: Moderate 414).
  • Consider endoscopic evaluation to assess the extent and impact on airway patency 67.
  • Employ embolization or laser therapy for severe cases causing significant obstruction or life-threatening complications (Evidence: Weak 1524).
  • Monitor for and manage complications such as anemia or thrombocytopenia, especially in cases with Kasabach-Merritt syndrome (Evidence: Expert opinion).
