Postprocedural aortic valve stenosis

Overview

Postprocedural aortic valve stenosis refers to the condition following aortic valve replacement (surgical or transcatheter) where residual stenosis or complications lead to impaired valve function. Transcatheter aortic valve implantation (TAVI) is increasingly used, particularly in high-risk patients 2.

Diagnosis

Management

  • Cardiac rehabilitation (CR) following TAVI is associated with significant improvements in exercise tolerance (e.g., Six-Minute Walk Test) and functional independence (e.g., Barthel Index) 2.
  • The use of renin-angiotensin system inhibitors (RASi) after aortic valve replacement (AVR), including TAVR, is associated with lower all-cause and cardiovascular mortality at short- and long-term follow-up 3.
  • Self-expandable valves (SEV) may be associated with a lower incidence of 30-day stroke after transfemoral TAVI compared to balloon-expandable valves (BEV) 4.
  • TAVR is associated with a lower risk of post-procedural patient-prosthesis mismatch (PPM) and improved effective orifice area (EOA) compared to surgical AVR (sAVR) 5.
  • TAVR is associated with a higher risk of moderate/severe paravalvular leak (PVL) compared to sAVR 5.
  • Machine learning models may offer improved predictive accuracy for all-cause mortality after TAVI compared to traditional risk scores 1.
