Overview

Pyoderma gangrenosum (PG) is an uncommon, neutrophilic, ulcerative skin disorder often characterized by painful skin lesions that can mimic other conditions such as venous ulcers or infections. It can occur in various clinical settings, including post-surgical wounds, chronic inflammatory diseases, and as a complication of certain therapies 1715.

Diagnosis

  • Clinical Features: Rapidly enlarging ulcers, painful nature, and lack of history of trauma 19.
  • Histopathological Criteria: Epidermotropism of inflammatory cells differentiates PG from venous leg ulcers 6.
  • Diagnostic Tools: Biopsy of ulcer margins showing specific histopathological features 6.
  • Diagnostic Scoring Systems: The PARACELSUS score can aid in diagnosing PG 9.
  • Differential Diagnosis: Important to rule out venous ulcers, infections, and other inflammatory conditions 19.
  • Management

  • First-Line Treatment: Prednisolone remains the primary initial therapy 2.
  • Adjunctive Therapies: Ciclosporin has shown efficacy compared to prednisolone 13 (Evidence: Moderate).
  • Other Treatments: Topical therapy, dapsone, and other immunosuppressants may be used 11.
  • Biologic Agents: Increasing use but limited evidence; access varies by healthcare system 2.
  • Steroid Therapy Considerations: Use cautiously in post-surgical settings due to potential immunosuppression 7.
  • Specific Cases: Rituximab use may be associated with PG as an adverse event 8 (Evidence: Weak).
  • Special Populations

  • Pediatrics: Rare but associated with underlying systemic diseases; data limited 10.
  • Comorbidities: PG can complicate conditions like PAPA syndrome and post-surgical states 37.
  • Elderly: Increased susceptibility to misdiagnosis due to overlapping symptoms with chronic conditions 18.
  • Key Recommendations

  • Initial Treatment with Prednisolone: First-line therapy for managing pyoderma gangrenosum 2 (Evidence: Moderate).
  • Consider Ciclosporin for Inadequate Response: Ciclosporin may be superior to prednisolone in refractory cases 13 (Evidence: Moderate).
  • Biopsy for Confirmation: Histopathological examination is crucial for definitive diagnosis 6 (Evidence: Moderate).
  • Monitor for Adverse Drug Reactions: Particularly with biologic agents like Rituximab 8 (Evidence: Weak).
  • Evaluate for Underlying Conditions: Especially in pediatric and atypical presentations 103 (Evidence: Moderate).
  • Use Diagnostic Scoring Systems: Implement tools like PARACELSUS for improved diagnostic accuracy 9 (Evidence: Moderate).
