End stage liver disease

Last edited: 4/13/2026

Overview

End-stage liver disease (ESLD) represents the final stage of chronic liver disease, characterized by severe liver dysfunction leading to multisystem organ failure and significantly impaired quality of life. Palliative care is increasingly recognized as essential for managing symptoms and improving outcomes in these patients 937.

Diagnosis

  • Clinical features include jaundice, ascites, hepatic encephalopathy, and coagulopathy.
  • Laboratory tests: Elevated liver enzymes, prolonged INR, low albumin, and elevated bilirubin levels.
  • Imaging: Ultrasound or CT to assess liver morphology and detect complications like portal hypertension.
  • Diagnostic criteria often involve the presence of decompensated liver function indicators and exclusion of other causes 37.
  • Management

  • Symptom Management: Focus on controlling ascites, hepatic encephalopathy, and pain. Use lactulose for encephalopathy, diuretics for ascites, and opioids for pain 3720.
  • Nutritional Support: Address malnutrition with tailored nutritional plans, considering the high risk of complications like hepatic osteodystrophy 37.
  • Palliative Care Integration: Early involvement of palliative care teams to address physical, psychological, and social needs 937.
  • Medications: Vitamin D supplementation to address deficiency, which is common and associated with poor outcomes 78.
  • End-of-Life Care: Concurrent hospice care can improve quality of life and reduce symptom burden; however, access barriers exist, particularly in veteran populations 61213.
  • Special Populations

  • Pregnancy: Women with ESLD are at significantly higher risk of adverse pregnancy outcomes and require careful multidisciplinary management 15.
  • Pediatrics: Management includes specialized nutritional support and access to pediatric-specific dialysis modalities 24.
  • Elderly: Increased frailty and comorbidity influence treatment decisions, emphasizing conservative management and palliative care 35.
  • Comorbidities: Patients often have concurrent conditions like cardiovascular disease, which require tailored management strategies 68.
  • Key Recommendations

  • Integrate palliative care early in the management of ESLD to address comprehensive patient needs (Evidence: Moderate 9).
  • Utilize concurrent hospice care to enhance end-of-life comfort and reduce symptom burden, despite existing access barriers (Evidence: Moderate 612).
  • Implement vitamin D supplementation to mitigate deficiencies common in ESLD patients, given its impact on outcomes (Evidence: Moderate 78).
  • Tailor nutritional support to individual patient needs, considering the high prevalence of malnutrition in ESLD (Evidence: Expert opinion 37).
  • Optimize symptom management, particularly for ascites, hepatic encephalopathy, and pain, using evidence-based pharmacological approaches (Evidence: Moderate 3720).
