Allergic gingival disease

Last edited: 4/13/2026

Overview

Allergic gingival disease, also known as allergic contact stomatitis or oral allergic contact dermatitis, involves inflammation of the gingiva triggered by hypersensitivity reactions to allergens such as dental materials, foods, or environmental factors. 3338

Diagnosis

  • Clinical presentation: Presence of erythematous, edematous, and sometimes ulcerated gingival lesions.
  • History: Identification of potential allergens through patient history, including recent dental procedures or new dietary exposures.
  • Patch testing: Useful for identifying specific allergens causing the reaction. 3338
  • Biopsy: In cases where diagnosis is unclear, histopathological examination can confirm allergic etiology. 33
  • Management

  • Avoidance of allergens: Identify and eliminate exposure to the specific allergen. 3338
  • Topical corticosteroids: Application of topical corticosteroids to reduce inflammation. 33
  • Antihistamines: Oral antihistamines for symptomatic relief, particularly for itching and discomfort. 33
  • Systemic corticosteroids: In severe cases, short-term systemic corticosteroids may be necessary. 33
  • Immunomodulatory agents: In refractory cases, consider immunomodulatory treatments under specialist guidance. 33
  • Special Populations

  • Pediatrics: Children may present with more generalized oral symptoms; careful history and patch testing are crucial. 33
  • Elderly: Increased prevalence of chronic conditions may complicate diagnosis and management; thorough evaluation is essential. 33
  • Key Recommendations

  • Identify and avoid specific allergens through detailed patient history and patch testing (Evidence: Moderate 3338).
  • Initiate treatment with topical corticosteroids for localized inflammation (Evidence: Moderate 33).
  • Consider systemic corticosteroids for severe, refractory cases (Evidence: Weak 33).
  • Monitor and manage comorbidities that may affect treatment outcomes (Evidence: Expert opinion 33).
