Erosive gastro-esophageal reflux disease

Last edited: 4/14/2026

Overview

Erosive gastro-esophageal reflux disease (GERD) is characterized by chronic acid reflux leading to mucosal damage in the esophagus, often manifesting as heartburn and regurgitation 1529.

Diagnosis

  • Symptom Assessment: Heartburn, regurgitation, dysphagia 1529.
  • Endoscopy: Visual confirmation of esophagitis and erosions 15.
  • pH Monitoring: 24-hour pH testing for acid exposure 3329.
  • Grading: Los Angeles Classification System for esophagitis severity 15.
  • Management

  • First-Line Treatments:
  • - Proton Pump Inhibitors (PPIs): Standard first-line therapy, dose varies but typically high (e.g., omeprazole 20-40 mg daily) 115. - H2 Receptor Antagonists: Alternative or adjunctive therapy 15.
  • Adjunctive Treatments:
  • - Lifestyle Modifications: Weight loss, dietary changes, elevating head of bed 15. - Surgical Intervention: For refractory cases, options include laparoscopic fundoplication (Nissen, anterior) 3620.

    Special Populations

  • Pregnancy: PPIs are generally considered safe but use should be minimized; alternatives like H2 blockers recommended 6.
  • Elderly: Increased risk of complications; careful monitoring and dose adjustment of PPIs 15.
  • Comorbidities: Consider impact of comorbidities on GERD management; Helicobacter pylori eradication does not exacerbate GERD 2627.
  • Key Recommendations

  • Initiate PPI therapy as first-line treatment for erosive GERD (Evidence: Strong 115).
  • Consider endoscopy for persistent symptoms despite PPI therapy to rule out complications (Evidence: Moderate 15).
  • Evaluate and manage comorbidities, such as obesity and Helicobacter pylori infection, which can influence GERD outcomes (Evidence: Moderate 1826).
  • Refer patients with refractory symptoms to surgical options like laparoscopic fundoplication after thorough evaluation (Evidence: Expert opinion 3620).
