Hereditary breast and ovarian cancer syndrome

Last edited: 4/23/2026

Overview

Hereditary breast and ovarian cancer syndrome (HBOC) is a genetic condition primarily associated with pathogenic variants in the BRCA1 and BRCA2 genes, leading to significantly increased risks of breast and ovarian cancers. 15

Diagnosis

  • Genetic testing for BRCA1 and BRCA2 mutations is essential for diagnosis. 15
  • Family history assessment for multiple cases of breast or ovarian cancer, especially at young ages, is crucial. 15
  • Clinical criteria such as the Amsterdam criteria or modified versions can guide genetic testing referrals. 15
  • Management

  • Risk-reducing salpingo-oophorectomy (RRSO): Recommended for women with BRCA1/2 mutations, typically between ages 35-40 or after childbearing is complete. 210
  • Hormone replacement therapy (HRT): Considered after RRSO to manage menopausal symptoms, with careful monitoring for potential risks. 30
  • Chemotherapy and targeted therapies: PARP inhibitors (e.g., olaparib, niraparib, rucaparib) are first-line treatments for BRCA-mutated ovarian cancer. 4914
  • Thromboprophylaxis: Extended duration recommended post-surgery to mitigate VTE risk. 2833
  • Special Populations

  • Pregnancy: Management requires careful consideration of cancer risk and potential teratogenic effects of treatments. 46
  • Elderly: Polypharmacy and comorbidities necessitate individualized treatment plans with close monitoring for adverse events. 2252
  • Comorbidities: Patients with HIV and cancer may require tailored antiretroviral and oncologic therapy to manage drug interactions. 6
  • Key Recommendations

  • Genetic testing for BRCA1/2 mutations should be offered to individuals with strong family histories of breast or ovarian cancer. (Evidence: Strong 15)
  • Risk-reducing salpingo-oophorectomy is recommended for women with BRCA1/2 mutations, ideally before age 40 or post-menopause. (Evidence: Strong 210)
  • PARP inhibitors are first-line treatments for BRCA-mutated ovarian cancer, offering improved outcomes. (Evidence: Strong 414)
  • Extended thromboprophylaxis should be considered post-surgery to reduce the risk of venous thromboembolism in ovarian cancer patients. (Evidence: Moderate 2833)
  • Close monitoring for adverse effects, particularly cardiovascular events and polyneuropathy, is essential in patients receiving PARP inhibitors. (Evidence: Moderate 78)
