Alcoholism

Last edited: 4/14/2026

Overview

Alcoholism, characterized by compulsive alcohol use despite harmful consequences, involves significant impairment in daily functioning and often coexists with nutritional deficiencies, particularly of B-complex vitamins 23.

Diagnosis

  • Clinical Criteria: Presence of tolerance, withdrawal symptoms, persistent desire or unsuccessful efforts to cut down or control use 12.
  • Physical Signs: Look for signs such as palmar erythema, spider angiomas, and signs of malnutrition 24.
  • Laboratory Tests: Assess for elevated liver enzymes, prothrombin time (PT) prolongation, and deficiencies in thiamine and other B-complex vitamins 35.
  • Psychological Assessment: Use standardized questionnaires like AUDIT (Alcohol Use Disorders Identification Test) 12.
  • Management

  • First-Line Treatments:
  • - Behavioral Therapies: Cognitive-behavioral therapy (CBT) and motivational interviewing 1522. - Nutritional Supplementation: Thiamine and other B-complex vitamins to address deficiencies 23.
  • Adjunctive Treatments:
  • - Medications: Disulfiram for aversion therapy, naltrexone for reducing cravings 26. - Family Therapy: Involve family members to address systemic impacts 22.

    Special Populations

  • Elderly: Increased focus on identification and diagnosis due to growing elderly alcoholic population 17.
  • Pediatrics: Limited direct evidence; family history and parental alcoholism significantly influence risk 10.
  • Comorbidities: Consider nutritional deficiencies and their impact on liver function and coagulation (e.g., prolonged PT due to acetaldehyde) 3.
  • Key Recommendations

  • Implement Nutritional Support: Supplement with thiamine and B-complex vitamins to address deficiencies common in alcoholics (Evidence: Strong 23).
  • Utilize Family Therapy: Engage family members in treatment to address systemic issues and support recovery (Evidence: Moderate 22).
  • Early Identification: Physicians should be vigilant for physical signs and behavioral clues indicative of alcoholism for early diagnosis (Evidence: Moderate 24).
  • Behavioral Interventions: Incorporate cognitive-behavioral therapy and motivational interviewing as core components of treatment (Evidence: Moderate 15).
  • Consider Medication: Use disulfiram or naltrexone as adjuncts based on patient suitability and clinical context (Evidence: Weak 26).
