Idiopathic glomerular disease

Last edited: 4 h ago

Overview

Idiopathic cutaneous and renal glomerular vasculopathy (CRGV), also known as Alabama rot, is a rare, potentially fatal condition affecting Greyhounds, characterized by cutaneous ulcers, thrombocytopenia, and acute renal insufficiency 1.

Diagnosis

  • Clinical Presentation: Characteristic cutaneous ulcers on extremities, thrombocytopenia (<200,000 platelets/μL), and acute renal insufficiency (BUN >40 mg/dL, serum creatinine >2.0 mg/dL) 1.
  • Imaging and Lab Tests: Renal function tests, complete blood count, and urinalysis to assess for renal impairment and hematological abnormalities 1.
  • Ultrastructural Examination: Glomerular ultrastructural changes including endothelial swelling, membranous whorl formation, and subendothelial accumulation of electron-dense material may aid in diagnosis 1.
  • Management

  • Supportive Care: Management focuses on supportive care including fluid therapy to manage renal failure and monitoring for complications 1.
  • Antithrombotic Therapy: Consideration of antithrombotic agents to address thrombocytopenia and microvascular thrombosis, though specific drug classes and doses are not detailed 1.
  • Dialysis: Renal replacement therapy may be necessary in cases of severe acute kidney injury 1.
  • Special Populations

  • Greyhounds Only: The condition is specific to Greyhounds, with no specific guidance provided for other breeds, age groups, or comorbidities 1.
  • Key Recommendations

  • Diagnose Based on Clinical and Laboratory Criteria: Confirm diagnosis using characteristic cutaneous ulcers, thrombocytopenia, and renal insufficiency markers (Evidence: Moderate 1).
  • Implement Supportive Renal and Hematologic Management: Focus on fluid therapy and monitoring for acute kidney injury and hematological abnormalities (Evidence: Moderate 1).
  • Consider Antithrombotic Therapy for Thrombocytopenia: Address microvascular thrombosis and thrombocytopenia with appropriate antithrombotic agents as indicated (Evidence: Weak 1).
