Hormone sensitive prostate cancer

Hormone-sensitive prostate cancer (HSPC) refers to prostate cancer that is initially responsive to androgen deprivation therapy (ADT), typically through surgical castration or pharmacological means such as gonadotropin-releasing hormone (GnRH) agonists or antagonists. Pathophysiology

Prostate cancer cells are dependent on androgens for growth and survival, primarily testosterone and dihydrotestosterone (DHT). In HSPC, the initial reduction of androgen levels through castration or ADT leads to a temporary response, often characterized by decreased tumor burden and alleviation of symptoms. However, resistance mechanisms can develop over time, including mutations in the androgen receptor (AR), amplification of the AR gene, and activation of alternative signaling pathways that bypass androgen dependence. These molecular changes enable cancer cells to continue proliferating despite low androgen levels, leading to castration-resistant prostate cancer (CRPC). Diagnosis

The diagnosis of hormone-sensitive prostate cancer typically involves a combination of clinical assessment, biomarker evaluation, and tissue analysis. Key diagnostic steps include:

  • Digital Rectal Examination (DRE): Identifies abnormalities in prostate size and texture.

  • Imaging Studies: MRI and bone scans help assess tumor extent and metastasis.
  • Histopathologic Grading: Gleason score and TNM staging provide prognostic information.

  • Differential Diagnosis:

  • Benign Prostatic Hyperplasia (BPH): Differentiated by absence of elevated PSA levels outside normal ranges and lack of malignant cells on biopsy.
  • Other Malignancies: Metastatic cancers to the prostate or other urological malignancies require careful histopathological differentiation.
  • Management

    First-Line Treatment

  • Androgen Deprivation Therapy (ADT):
