Acquired ataxia

Last edited: 4/14/2026

Overview

Acquired ataxia refers to a neurological disorder characterized by a lack of muscle control during voluntary movements, such as walking or picking up objects, often resulting from damage to the cerebellum or its connections. It can be secondary to various conditions including neurodegenerative diseases, metabolic disorders, and certain vitamin deficiencies 811.

Diagnosis

  • Clinical evaluation focusing on gait disturbances, coordination issues, and speech abnormalities (dysarthria) 23.
  • Neuroimaging (MRI) to identify structural abnormalities 3.
  • Laboratory tests to rule out metabolic or vitamin deficiencies, such as measuring α-tocopherol levels 811.
  • Genetic testing for specific mutations associated with ataxia syndromes 15.
  • Management

  • Speech Therapy: Intensive home-based speech training programs tailored to cerebellar dysarthria have shown promise in improving intelligibility and naturalness 23.
  • Vitamin Supplementation: Coenzyme Q(10) supplementation for patients with CoQ(10) deficiency has demonstrated clinical improvement over 2 years 11.
  • Supportive Care: Addressing fear of falling and providing physical therapy to maintain mobility and reduce fall risk 9.
  • Special Populations

  • Pediatrics: Specific genetic mutations like Senataxin mutations can present early in life with cerebellar ataxia and oculomotor defects 15.
  • Elderly: Increased susceptibility to complications from falls and mobility issues; tailored rehabilitation programs are crucial 9.
  • Key Recommendations

  • Implement intensive speech therapy programs for patients with acquired ataxia to enhance communication abilities (Evidence: Moderate 23).
  • Consider coenzyme Q(10) supplementation in patients identified with CoQ(10) deficiency to manage ataxia symptoms (Evidence: Strong 11).
  • Provide comprehensive supportive care, including physical therapy and strategies to mitigate fear of falling, especially in elderly patients (Evidence: Expert opinion 9).
