Chronic venous infarction of spinal cord

Last edited: 4/23/2026

Overview

Chronic venous infarction of the spinal cord is a rare condition characterized by localized damage to the spinal cord due to venous congestion and thrombosis, leading to neurological deficits and potentially irreversible damage if not promptly addressed. 20

Diagnosis

  • Clinical Presentation: Neurological deficits corresponding to the level of spinal cord involvement.
  • Imaging: MRI is crucial for identifying areas of infarction and distinguishing from other spinal cord pathologies.
  • Venography: May be used to visualize venous abnormalities, though invasive.
  • Grading: Utilization of the American Spinal Injury Association (ASIA) Impairment Scale to assess severity and monitor progression. 20
  • Management

  • Early Intervention: Urgent decompression and stabilization to prevent further neurological deterioration.
  • Thrombolysis/Thrombectomy: Consideration in acute cases to restore blood flow, though evidence is limited and specific to venous thrombosis in other contexts 20.
  • Supportive Care: Management of pain, prevention of complications (e.g., deep vein thrombosis), and rehabilitation.
  • Compression Therapy: Not typically applicable due to spinal cord involvement but crucial in peripheral chronic venous disease management 416.
  • Special Populations

  • Pregnancy: Limited data; management focuses on conservative care and monitoring due to risks associated with invasive procedures 30.
  • Elderly: Increased risk of complications; individualized treatment plans considering comorbidities and functional status 518.
  • Key Recommendations

  • Immediate MRI Evaluation: Essential for diagnosis and guiding treatment decisions (Evidence: Expert opinion 20).
  • Urgent Neurosurgical Consultation: For potential decompression and stabilization in acute cases (Evidence: Expert opinion 20).
  • Multidisciplinary Approach: Including neurology, vascular surgery, and rehabilitation for comprehensive care (Evidence: Expert opinion 20).
  • Avoid Invasive Procedures in Chronic Stable Cases: Focus on supportive care and symptom management due to high risk (Evidence: Expert opinion 20).
  • Monitor for Complications: Regular assessment for deep vein thrombosis and other secondary complications (Evidence: Expert opinion 20).
