Autosomal dominant striatal neurodegeneration

Last edited: 4/14/2026

Overview

Autosomal dominant striatal neurodegeneration (ADSD) is a rare genetic disorder characterized by progressive motor dysfunction primarily affecting the striatum, leading to symptoms such as chorea, dystonia, and cognitive decline. It often manifests in adulthood and is associated with mutations in specific genes affecting neuronal function.

Diagnosis

  • Clinical presentation of progressive chorea, dystonia, and cognitive impairment 25.
  • Genetic testing identifying mutations in genes such as ATXN1, ATXN2, or ATXN3 associated with spinocerebellar ataxias (SCAs) that can overlap with ADSD 6.
  • Neuroimaging showing striatal atrophy or signal changes 25.
  • Exclusion of other causes of movement disorders through comprehensive neurological evaluation 25.
  • Management

  • Symptomatic treatment: Use of dopaminergic agents (e.g., levodopa) to manage motor symptoms 25.
  • Supportive therapies: Physical therapy and occupational therapy to maintain function 25.
  • Cognitive support: Cognitive rehabilitation and management of cognitive decline 25.
  • Monitoring: Regular neurological assessments to track disease progression 25.
  • Special Populations

  • Elderly: Increased susceptibility to neurodegeneration and potential drug interactions; careful monitoring of medication efficacy and side effects 16.
  • Comorbidities: Management of comorbidities like cardiovascular disease may influence overall prognosis and treatment tolerance 16.
  • Key Recommendations

  • Genetic testing for definitive diagnosis and family screening 6 (Evidence: Strong).
  • Initiate symptomatic treatment with dopaminergic agents early in the disease course to manage motor symptoms effectively 25 (Evidence: Moderate).
  • Regular multidisciplinary assessments including neurology, physical therapy, and cognitive support to tailor management strategies 25 (Evidence: Moderate).
  • Consider cognitive rehabilitation for patients experiencing cognitive decline to maintain quality of life 25 (Evidence: Weak).
  • Monitor for drug interactions and adjust medications accordingly in elderly patients with comorbidities 16 (Evidence: Expert opinion).
