Acromegaly

Last edited: 4/23/2026

Overview

Acromegaly is a rare endocrine disorder characterized by excessive growth hormone (GH) and insulin-like growth factor 1 (IGF-1) production, typically due to a pituitary adenoma, leading to various systemic manifestations including skeletal, soft tissue, and cardiovascular abnormalities 1215.

Diagnosis

  • Key Diagnostic Criteria: Elevated IGF-1 levels, confirmatory GH suppression test 1.
  • Recommended Tests: MRI of the pituitary gland to identify the adenoma 2.
  • Cardiovascular Assessment: Evaluate for left ventricular hypertrophy, arterial stiffness, and carotid intima-media thickness 41217.
  • Skin Examination: Look for cutaneous changes such as hyperpigmentation, skin tags, and acrochordons 10.
  • Management

  • First-Line Treatments:
  • - Somatostatin Analogues: Octreotide, lanreotide, pasireotide 32324. - Dopamine Agonists: Cabergoline for smaller tumors 23.
  • Surgery: Transsphenoidal resection of the pituitary adenoma 1924.
  • Radiation Therapy: For persistent disease post-surgery 1924.
  • GH Receptor Antagonists: Pegvisomant for achieving biochemical control 71.
  • Special Populations

  • Pregnancy: Evaluate disease activity, manage with medical therapy, and monitor maternal and fetal outcomes closely 9.
  • Comorbidities: Focus on cardiovascular risk factors, including hypertension, left ventricular hypertrophy, and endothelial dysfunction 12131417.
  • Elderly: Tailor management considering comorbidities and potential drug interactions 24.
  • Key Recommendations

  • Achieve biochemical control through normalization of IGF-1 and GH levels to reduce comorbidities (Evidence: Strong 21).
  • Regular cardiovascular monitoring is essential due to increased risk of heart disease, including left ventricular hypertrophy and arterial stiffness (Evidence: Moderate 121314).
  • Multidisciplinary team management is recommended for newly diagnosed patients to optimize treatment outcomes (Evidence: Expert opinion 19).
  • Evaluate and manage skin manifestations, including autofluorescence and dermatoscopic changes, to assess disease activity (Evidence: Moderate 10).
  • Consider cumulative GH exposure in assessing long-term morbidity and mortality risks (Evidence: Moderate 45).
