Cholecystitis

Overview

Acute cholecystitis is an inflammatory condition of the gallbladder, often caused by gallstones, leading to symptoms such as right upper quadrant pain, fever, and leukocytosis. It can range from mild to severe, with complications including gallbladder perforation and empyema. 629

Diagnosis

  • Clinical Criteria: Right upper quadrant pain, fever, nausea, vomiting, and leukocytosis.
  • Imaging: Ultrasound is the primary imaging modality, showing gallstones, thickened gallbladder wall (≥3 mm), pericholecystic fluid, and sometimes gall bladder wall edema.
  • Grading: Utilize imaging findings and clinical severity to grade severity (e.g., American Association for the Surgery of Trauma [AAST] grading system). 225
  • Management

  • First-Line Treatment: Emergency laparoscopic cholecystectomy is recommended for most patients, especially those with severe or complicated cholecystitis. 919
  • Medical Management: For high-risk patients, percutaneous cholecystostomy may be considered as a bridge to definitive surgery. 2619
  • Antibiotics: Broad-spectrum antibiotics should be initiated preoperatively to cover common pathogens like E. coli, Bacteroides, and Clostridium. 647
  • Pain Management: Adequate analgesia is crucial, often managed with intravenous opioids initially followed by oral analgesics. 51

  • Elderly: Emergency laparoscopic cholecystectomy remains superior, but frailty indices and nutritional status (e.g., Geriatric Nutritional Risk Index) should guide risk stratification. 1423
  • Pregnancy: Diagnostic imaging should prioritize safety for the fetus; management often involves conservative treatment initially, with surgery deferred if possible. 648
  • Comorbidities: Patients with multiple comorbidities may benefit from percutaneous cholecystostomy as a less invasive initial approach. 1926
