Dermatofibrosarcoma protuberans

Last edited: 4/14/2026

Overview

Dermatofibrosarcoma protuberans (DFSP) is a rare, locally aggressive fibroblastic tumor of the skin with low metastatic potential but high recurrence rates if incompletely resected 15.

Diagnosis

  • Clinical suspicion based on characteristic growth patterns
  • Histopathological confirmation essential; biopsy required 15
  • Chromosomal translocations (e.g., t(17;22)) and fusion gene transcripts (e.g., EWSR1-NR4A3) aid in diagnosis 1
  • Misdiagnosis common, often mistaken for benign lesions 2
  • Management

  • Primary treatment: Complete surgical excision with adequate margins
  • - Microscopically controlled surgery recommended to minimize recurrence 14 - Standard excision with lateral margins of 2-3 cm acceptable if microscopic control unavailable 1
  • Adjunctive therapy:
  • - Imatinib: Approved for advanced or metastatic DFSP; objective response rates ~50% 56 - Radiation therapy for inoperable or metastatic cases 5

    Special Populations

  • Pregnancy: No specific guidelines provided in abstracts 78910
  • Pediatrics: No specific data provided in abstracts 78910
  • Elderly: No specific considerations noted in abstracts 78910
  • Comorbidities: Management similar regardless of comorbidities; focus on complete resection 15
  • Key Recommendations

  • Surgical excision with margins ≥3 cm significantly reduces recurrence rates (Evidence: Strong 4)
  • Microscopically controlled surgery is preferred to ensure complete tumor removal (Evidence: Moderate 1)
  • Imatinib should be considered for advanced or metastatic DFSP cases (Evidence: Moderate 56)
  • Early dermatology consultation improves appropriate diagnosis and treatment outcomes (Evidence: Moderate 2)
  • Continuous cervical erector spinae plane block can be effective for perioperative analgesia in extensive DFSP surgeries (Evidence: Weak 3)
