Tetanus

Last edited: 4/13/2026

Overview

Tetanus is a serious, often fatal, disease caused by the toxin produced by Clostridium tetani, leading to muscle stiffness and spasms, particularly in unvaccinated or inadequately immunized populations 115.

Diagnosis

  • Clinical Presentation: Muscle stiffness, particularly jaw (trismus) and neck stiffness, followed by generalized spasms 15.
  • Wound Assessment: Identify and clean any wounds that may be contaminated with C. tetani spores 15.
  • Laboratory Tests: Not typically required for diagnosis but may include tetanus antibody levels in severe cases 15.
  • Grading: Based on clinical severity, ranging from localized spasms to generalized muscle rigidity and autonomic dysfunction 15.
  • Management

  • Antibiotics: Penicillin or metronidazole to reduce toxin production 15.
  • Tetanus Immune Globulin (TIG): Administered intramuscularly for unimmunized or incompletely immunized patients 5.
  • Human Antitoxin: Given intravenously to neutralize circulating toxin 15.
  • Wound Care: Thorough cleaning and debridement of wounds 15.
  • Supportive Care: Mechanical ventilation, sedation (e.g., diazepam, morphine), and neuromuscular blockade (e.g., dantrolene) for severe cases 1737.
  • Pain Management: Use of analgesics and sedation to manage muscle spasms 17.
  • Special Populations

  • Pregnancy: Maternal tetanus vaccination is crucial to prevent neonatal tetanus; Tdap vaccination recommended during pregnancy 6.
  • Elderly: Higher mortality rates; ensure complete immunization status and prompt wound care 2242.
  • Comorbidities: Patients with burns or other severe wounds require meticulous tetanus prophylaxis 47.
  • Key Recommendations

  • Vaccination: Ensure complete tetanus vaccination series, including booster doses every 10 years for adults 1611(Evidence: Strong).
  • Wound Management: Clean and debride wounds promptly to prevent tetanus onset 15(Evidence: Strong).
  • Prophylaxis in Wound Cases: Administer TIG and antibiotics to unvaccinated or incompletely immunized individuals with contaminated wounds 5(Evidence: Moderate).
  • Supportive Care: Use mechanical ventilation and neuromuscular blockers for severe cases 17(Evidence: Moderate).
  • Monitoring and Education: Regularly assess tetanus knowledge and practices in emergency departments to improve prophylaxis adherence 23(Evidence: Moderate).
