Malabsorption in the elderly

Last edited: 4/23/2026

Overview

Malabsorption in the elderly refers to impaired nutrient absorption, often exacerbated by age-related physiological changes, polypharmacy, and comorbid conditions such as gastrointestinal disorders, malnutrition, and cognitive impairment. 939

Diagnosis

  • Clinical Symptoms: Diarrhea, weight loss, nutrient deficiencies (e.g., anemia, vitamin B12 deficiency). 9
  • Laboratory Tests: Fecal fat test, serum vitamin levels, complete blood count (CBC). 9
  • Endoscopy and Biopsy: To assess for structural abnormalities in the gastrointestinal tract. 9
  • Stool Analysis: For fat content and pathogens. 9
  • Management

  • Dietary Modifications: High-protein, high-calorie diet with nutrient-dense foods; consider supplements as needed. 9
  • Address Polypharmacy: Review and deprescribe potentially inappropriate medications (PIMs) using tools like STOPP/START criteria. 71830
  • Nutritional Support: Enteral feeding if severe malnutrition persists despite dietary interventions. 9
  • Management of Underlying Conditions: Treat specific causes such as celiac disease, inflammatory bowel disease, or infections. 9
  • Special Populations

  • Elderly: Increased risk due to age-related changes in gut physiology, higher prevalence of comorbidities, and polypharmacy. 939
  • Comorbidities: Cognitive impairment and functional decline can complicate diagnosis and management. 39
  • Key Recommendations

  • Conduct Comprehensive Medication Review: Regularly assess and deprescribe PIMs to reduce anticholinergic burden and improve absorption. (Evidence: Moderate 718)
  • Implement Nutritional Assessments: Regularly evaluate nutritional status and provide targeted supplementation as needed. (Evidence: Moderate 9)
  • Utilize Explicit Deprescribing Tools: Apply validated tools like STOPP/START criteria to guide deprescribing efforts in elderly patients. (Evidence: Moderate 1830)
  • Monitor for Adverse Drug Interactions: Screen for potential drug-drug interactions that may exacerbate malabsorption. (Evidence: Weak 64)
  • Engage Multidisciplinary Teams: Involve pharmacists, dietitians, and geriatricians in managing complex cases of malabsorption. (Evidence: Expert opinion 60)
