Burkitt lymphoma

Overview

Burkitt lymphoma is a rare, aggressive subtype of non-Hodgkin lymphoma, accounting for less than 5% of adult lymphomas. It is characterized by rapid progression and high incidence of central nervous system (CNS) involvement and tumor lysis syndrome, especially in patients with high tumor burden 1.

Diagnosis

  • Clinical Presentation: Rapidly enlarging lymph nodes, abdominal masses, and symptoms related to organ involvement 167.
  • Imaging: CT scans and MRI for assessing extent of disease and CNS involvement 1.
  • Biopsy: Essential for definitive diagnosis; histopathology and immunohistochemistry confirm the diagnosis 1.
  • CNS Evaluation: Lumbar puncture and cerebrospinal fluid analysis to assess for CNS involvement 1.
  • Laboratory Tests: Complete blood count often shows marked anemia and thrombocytopenia 13.
  • Tumor Burden Assessment: Important for risk stratification and management decisions 1.
  • Management

  • First-Line Treatment: Dose-dense chemotherapy regimens, often incorporating protocols like CODOX-M/IVAC or variants 14.
  • CNS Prophylaxis: Intrathecal chemotherapy or high-dose systemic methotrexate to prevent CNS relapse 1.
  • Tumor Lysis Syndrome Prevention: Aggressive hydration, allopurinol, and monitoring electrolytes 1.
  • Supportive Care: Management of infections, anemia, and other complications; transfusion support as needed 1.
  • Radiation Therapy: Used for localized disease or bulky disease to improve local control 12.
  • Consolidation/Maintenance Therapy: Consideration based on risk stratification and institutional protocols 4.
  • Special Populations

  • Pediatrics: Similar aggressive treatment approaches as adults but with careful consideration of developmental impacts 6.
  • Comorbidities: Management tailored to address concurrent conditions like severe anemia or sepsis 713.
  • Developing Countries: Challenges in sustainability and access to intensive chemotherapy regimens noted 10.
  • Key Recommendations

  • Use Dose-Dense Chemotherapy Regimens for optimal outcomes in adult Burkitt lymphoma patients (Evidence: Strong 14).
  • Implement CNS Prophylaxis to reduce risk of central nervous system relapse (Evidence: Strong 1).
  • Monitor and Manage Tumor Lysis Syndrome Proactively to prevent life-threatening complications (Evidence: Moderate 1).
  • Consider Individualized Risk Stratification for treatment intensity and supportive care needs (Evidence: Moderate 2).
  • Integrate Palliative and Psychosocial Support for comprehensive care, especially in complex cases (Evidence: Expert opinion 3).
