Flail chest

Last edited: 4/23/2026

Overview

Flail chest is a traumatic injury characterized by multiple rib fractures that result in paradoxical chest wall motion, often complicating respiratory function and potentially leading to respiratory failure and increased morbidity 6668.

Diagnosis

  • Clinical Presentation: Presence of paradoxical chest wall movement, respiratory distress, and pain 66.
  • Imaging: Chest X-ray is typically the initial imaging modality to identify fractures and flail segment 66.
  • CT Scan: Provides detailed visualization of fractures and associated injuries 66.
  • Laboratory Tests: Not specific but may show signs of systemic inflammation or complications 66.
  • Management

  • Respiratory Support: Mechanical ventilation may be required for severe cases 66.
  • Pain Management: Analgesics such as opioids for pain control 7.
  • Surgical Fixation: Operative fixation of fractures can improve respiratory function and reduce analgesia needs 7.
  • Monitoring: Close monitoring for complications like pneumonia, atelectasis, and respiratory failure 66.
  • Special Populations

  • Pediatrics: Management focuses on minimizing pain and ensuring adequate ventilation; surgical fixation may be considered for severe cases 35.
  • Elderly: Increased risk of complications; tailored respiratory support and pain management are crucial 66.
  • Comorbidities: Presence of pre-existing respiratory conditions may necessitate more aggressive respiratory support 66.
  • Key Recommendations

  • Imaging for Diagnosis: Utilize chest X-ray initially, followed by CT scan for detailed assessment of fractures and associated injuries (Evidence: Moderate 66).
  • Respiratory Support: Provide mechanical ventilation for patients with severe respiratory compromise (Evidence: Moderate 66).
  • Surgical Fixation: Consider operative fixation of fractures in patients with significant flail segments to improve respiratory function and reduce analgesia requirements (Evidence: Moderate 7).
  • Pain Control: Implement aggressive pain management strategies using opioids to facilitate breathing and patient comfort (Evidence: Expert opinion 7).
  • Close Monitoring: Monitor for and manage complications such as pneumonia and respiratory failure (Evidence: Moderate 66).
