Nodule of lung

Last edited: 4/14/2026

Overview

A pulmonary nodule is an abnormal growth in the lung tissue, often detected incidentally on imaging studies like CT scans. These nodules can be benign or malignant and require careful evaluation to determine their nature and management strategy.

Diagnosis

  • Initial Imaging: CT scan with contrast and PET scans for characterization 12.
  • Biopsy Techniques:
  • - CT-guided transthoracic needle aspiration (TTNA) for nodules near chest wall or deeper lesions 1. - Bronchoscopic approaches (EBUS, ENB, VBN) for nodules near patent bronchi 1. - Ultrasound-guided TTNA for nodules in contact with pleura 1.
  • Respiratory Gating: Enhances diagnostic accuracy for small nodules (<15 mm) 7.
  • Artifact Awareness: Recognize potential ECG electrode artifacts mimicking pulmonary nodules on radiographs 8.
  • Management

  • Follow-Up: Recommended for small nodules with size thresholds and intervals adjusted based on risk factors 2.
  • Biopsy: Preferred for nodules ≥2 cm or in high-risk patients for pneumothorax 1.
  • Surveillance vs. Intervention: Decisions guided by malignancy likelihood, patient suitability for invasive procedures, and patient preferences 3.
  • Surgical Options: Video-assisted thoracoscopic surgery (VATS) for definitive diagnosis and treatment in selected cases 5.
  • Special Populations

  • Patient Preferences: Engage in shared decision-making, particularly when patient preferences significantly influence management 3.
  • No Specific Guidelines: Abstracts do not provide detailed recommendations for pregnancy, pediatrics, or elderly populations specifically.
  • Key Recommendations

  • Assess Likelihood of Malignancy using imaging studies like CT and PET scans (Evidence: Strong 2).
  • Evaluate Patient Suitability for Invasive Procedures before proceeding with biopsy or surgery (Evidence: Strong 3).
  • Consider Patient Preferences in Decision-Making to tailor management strategies (Evidence: Moderate 3).
  • Use Respiratory Gating for Biopsy of Small Nodules to improve diagnostic accuracy (Evidence: Moderate 7).
  • Follow Evidence-Based Guidelines for Follow-Up Intervals, adjusting based on individual risk factors (Evidence: Moderate 2).
