Drug-induced lupus erythematosus

Last edited: 4/16/2026

Overview

Drug-induced lupus erythematosus (DILE) is a syndrome characterized by the development of autoantibodies and clinical manifestations similar to systemic lupus erythematosus (SLE) following exposure to certain medications, such as nitrofurantoin 1.

Diagnosis

  • Presence of clinical symptoms resembling SLE
  • Detection of autoantibodies (e.g., ANA, anti-dsDNA)
  • Temporal association with drug exposure
  • Exclusion of other causes of lupus-like symptoms
  • Serologic remission following drug withdrawal 1
  • Management

  • Discontinuation of the offending drug 1
  • Monitoring for clinical improvement and serologic changes
  • Symptomatic treatment for specific manifestations (e.g., NSAIDs for arthritis)
  • Corticosteroids if severe symptoms persist 1
  • Special Populations

  • Pregnancy: Limited data; careful monitoring and avoidance of teratogenic drugs 1
  • Elderly: Increased vigilance due to potential comorbidities and drug interactions 1
  • Key Recommendations

  • Withdraw the suspected offending drug immediately upon diagnosis of DILE to facilitate clinical and serologic remission (Evidence: Strong 1)
  • Monitor patients for clinical improvement and serologic changes post-drug withdrawal to confirm remission (Evidence: Moderate 1)
  • Consider symptomatic treatment and corticosteroids for severe manifestations, tailored to individual patient needs (Evidence: Expert opinion 1)
