Dental caries

Last edited: 4/13/2026

Overview

Dental caries, also known as tooth decay, is a multifactorial disease characterized by enamel demineralization caused by bacterial activity, particularly from Streptococcus mutans, leading to cavities and potential complications if left untreated 171122.

Diagnosis

  • Clinical examination for visible cavities, toothache, or sensitivity 17.
  • Radiographic assessment (X-rays) to detect caries not visible to the naked eye 17.
  • Caries risk assessment tools to evaluate risk factors 17.
  • Microbiological testing for specific pathogens like Streptococcus mutans 17.
  • Management

  • First-line treatments:
  • - Fluoride therapy (topical and systemic) to remineralize enamel 666. - Restorative procedures (fillings) for carious lesions 19.
  • Adjunctive treatments:
  • - Dietary counseling to reduce sugar intake 11334. - Sealants to protect pits and fissures from decay 113. - Antibacterial agents (e.g., chlorhexidine) for high-risk patients 226.

    Special Populations

  • Pediatrics:
  • - Use of behavior management techniques, conscious sedation, or general anesthesia for extensive treatment 39203945. - Emphasis on preventive care and parental compliance 3959.
  • Pregnancy:
  • - Prenatal vitamin D status influences enamel defects and tooth decay risk in children 122122. - Adequate maternal nutrition, particularly vitamin D, is crucial 122122.
  • Elderly:
  • - Increased risk due to medication use and reduced salivary flow 6768. - Regular dental check-ups and management of dry mouth symptoms 6768.

    Key Recommendations

  • Implement fluoride therapy as a primary preventive measure against dental caries (Evidence: Strong 666).
  • Conduct regular radiographic assessments to identify caries not visible clinically (Evidence: Moderate 1).
  • Provide dietary counseling focused on reducing sugar intake for all age groups (Evidence: Moderate 11334).
  • Use sealants in children and adolescents to prevent occlusal caries (Evidence: Moderate 113).
  • For pediatric patients requiring extensive treatment, consider behavior management, conscious sedation, or general anesthesia based on clinical need (Evidence: Moderate 39203945).
  • Monitor and address vitamin D levels in pregnant women to mitigate caries risk in offspring (Evidence: Moderate 122122).
  • Elderly patients should receive tailored care addressing medication-induced dry mouth and increased caries risk (Evidence: Moderate 6768).
