Organic mood disorder of mixed type

Last edited: 4/10/2026

Overview

Organic mood disorder of mixed type is characterized by mood fluctuations that are not attributable to another mental disorder and are due to the direct physiological effects of a general medical condition 1. This condition involves symptoms of both mania and depression occurring nearly simultaneously or in rapid sequence 1.

Diagnosis

  • Diagnosis requires evidence of the etiological direct physiological consequence of a general medical condition 1.
  • Symptoms must be clinically significant and cause marked distress or impairment in social, occupational, or other important areas of functioning 1.
  • Management

  • Sleep deprivation (SD) has shown an overall response rate of approximately 47.6% in the acute treatment of bipolar depression 1.
  • Adjunctive pharmacotherapy significantly increased response rates to SD, with patients using adjunctive medication showing a 59.4% response rate compared to 27.4% in those not using medication 1.
  • Key Recommendations

  • Sleep deprivation can be considered as an antidepressant intervention for bipolar depression, with an overall response rate of almost 50% 1. (Evidence: Moderate)
  • The use of adjunctive pharmacotherapy should be routinely paired with sleep deprivation interventions for bipolar depression, as it significantly moderates response rates 1. (Evidence: Moderate)
